It is with a heavy heart that we announce the closure of Origins Birth and Wellness Collective. This decision was not made lightly, and we deeply value the trust and support you’ve shown us throughout the years.

We are working to get all of our medical records downloaded and stored with a company who will maintain them. Once this process is complete we will post a link here for you to request them directly.

If your healthcare provider needs your medical records (or your child's medical records) before then, please have them email a signed records release form to midwife@originsbirth.com.
